*full name (both full names for couples' requests)

*proposed date and time-frame

*desired length and type of session 

 

Follow-up|

*Additional verification will be requested.  Please be prepared to screen with real world information.  This is non-negotiable!

*Deposit of 20% of the booking fee payable via cashapp or gift card.  Appointment time is not secured until a deposit is received.
